Viewing the error logs, I noticed that it said to run mysql_upgrade. I had code that used to work, and then failed recently, so I'm guessing it was a recent addition. Roundcube 1.0.1 has been installed successfully. Did that and my create user statement worked fine!

I must have been picking up something extra, maybe a space character.
